FIRST VISIT
Please contact your referring physician’s office for a referral note, to get any recent lab work, test results, or clinical notes. You will need these records for your first appointment. Please have them faxed to our office.

Office Policies:
Payment
Bring your insurance card to every visit. All copays are due at the time of your visit. If you are not insured, we require payment at the time of service. We accept cash, check, Visa, Mastercard, and Discover as forms of payment.
Cancellation Policy
Please notify us at least 24 hours prior to your appointment to cancel or change your appointment. If you do not show up for your scheduled appointment and did not cancel, you will be subject to cancellation fees.
Patient Responsibility
As the patient, it will be your responsibility to have your referral (if required) with you at the time of your visit. If you are unsure if a referral is necessary, you can check with your insurance company.
It is your responsibility to notify the office of any changes to your address, phone number, or any change in insurance prior to your appointment.
